WAIVERS AND LIABILITY FORMS

The form repository is where we keep all important documentation related to our staff as participants.  Please select from the appropriate buttons below and complete any required forms PRIOR to our events.  The safety of our participants is paramount but no operations will start without the proper forms being viewed (safety video) or digitally signed (waiver).

THE FOLLOWING ITEMS BELOW ARE REQUIRED PRIOR TO IN-PROCESSING

Our in processing team will validate that your name is on the digital waiver master list , so we ask you do it a few hours in advance so that you’re on the list. If you haven’t done the digital waiver you will be required to do so before you finish in processing. You do NOT need a copy of your signed waiver.

If you have discussed with the cadre about using your vehicle for the event, you must read and agree to the following waiver. We must have this on file prior to event start. NOTE: Not all vehicles will be approved for use.
